El tumor fibroso solitario (TFS) es una neoplasia mesenquimatosa de tipo fibroblástico que, a pesar de ser localizado principalmente en pleura, se ha observado en otros órganos como la próstata. Por su parte, el tumor fibroso solitario de la próstata es una neoplasia de baja incidencia, crecimiento lento y potencial maligno incierto, que generalmente se compone de células fusiformes de apariencia citológicamente benignas, dispuestas en una arquitectura desorganizada, mezcladas con colágeno y pequeños vasos sanguíneos. Establecer su diagnóstico se ha vuelto más reproducible desde la identificación de la fusión de los genes NAB2-STAT6 por biología molecular, que lleva a la sobreexpresión de STAT6 por inmunohistoquímica, el cual es un marcador muy sensible y específico para TFS. Presentamos el caso clínico de un paciente que debutó con síntomas de compresión vesical, en quien se identificó una masa con epicentro en la próstata que infiltraba la vejiga y llegaba a la pared rectal, y que luego de estudios de patología, inmunohistoquímica y pruebas moleculares se clasificó como un TFS de la próstata, finalmente tratado con cistoprostatectomía radical más derivación urinaria


Solitary fibrous tumor (SFT) is a mesenchymal neoplasm of fibroblastic type, which despite being located mainly in the pleura, has been observed in other organs such as the prostate. On the other hand, solitary fibrous tumor of the prostate is a rare neoplasm, slow growing, and of uncertain malignant potential, which is generally composed of spindle cells of cytologically benign appearance, arranged in a disorganized architecture, mixed with collagen and small blood vessels. Establishing its diagnosis has become more reproducible since the identification of the NAB2-STAT6 gene fusion by molecular biology, leading to the overexpression of STAT6 by immunohistochemistry, a very sensitive and specific marker for SFT. We present a clinical report of a patient who consulted with symptoms of bladder compression, in whom a mass was identified with the epicenter in the prostate infiltrating into the bladder and reaching the rectal wall. Following histopathology study, immunohistochemistry and molecular tests it was classified as a SFT of the prostate, finally treated with radical cystoprostatectomy plus urinary shunt

El struma ovarii es un tumor bastante inusual, hallado en el 2,7 % de los teratomas de ovario. Su variedad quística es bastante rara, siendo su diagnóstico diferencial un cistoadenoma de ovario. Se caracteriza por presentar zona sólida con folículos tiroideos con coloide en su interior, y una zona quística tapizada por un epitelio plano a cuboidal, ambos con marcación para TTF1. Presentamos el caso clínico de una paciente que fue remitida a nuestro hospital por presentar una masa a la palpación vaginal. En imágenes se observa la presencia de una masa anexial derecha quística, por lo cual se le realizó una histerectomía radical con salpingooferectomía bilateral. Luego de estudios de patología y de inmunohistoquímica, se llegó a la conclusión que correspondía a un struma ovarii quístico


Struma ovarii is a rather unusual tumor, found in 2.7% of ovarian teratomas. Its cystic variety is quite rare, with its differential diagnosis being an ovarian cystadenoma. It is characterized by the presence of a solid area with thyroid follicles with colloid inside, and a cystic area lined by a flat to cuboidal epithelium, both with TTF1 marking. We describe the case report of a patient referred to our hospital for presenting a mass on vaginal examination. Imaging studies revealed the presence of a right adnexal cystic mass, for which she underwent a radical hysterectomy with bilateral salpingo-oophorectomy. After pathology and immunohistochemistry studies, it was concluded that it corresponded to a cystic struma ovarii

Las lesiones metastásicas representan hasta un 3 % de los tumores malignos de la glándula tiroides. La mayoría de los casos se originan de tumores de células renales y de pulmón. El abordaje diagnóstico implica una alta sospecha clínica en pacientes con primarios conocidos, sin embargo, puede ser la manifestación inicial de una enfermedad maligna extensa no diagnosticada hasta en un 20 % a 40 % de los pacientes. La biopsia por aguja fina ha demostrado buen rendimiento para el diagnóstico de los nódulos metastásicos. El pronóstico y la opción del tratamiento quirúrgico dependen del control local del primario y del estado de la enfermedad sistémica asociada, por lo tanto, debe ser individualizado. Por lo general, hasta un 80 % de los pacientes con compromiso de la tiroides tienen enfermedad metastásica multiorgánica, y la intención del tratamiento quirúrgico es con fines paliativos para prevenir las complicaciones derivadas de la extensión local de la enfermedad a las estructuras del tracto aerodigestivo superior en el cuello. Se presenta a continuación, una serie de seis casos de pacientes con lesiones metastásicas a glándula tiroides con primarios en riñón, mama y de melanomas


Metastatic lesions represent up to 3% of malignant tumors of the thyroid gland. Most cases originate from lung and renal cell tumors. The diagnostic approach implies a high clinical suspicion in patients with known primaries, however, it can be the initial manifestation of an extensive undiagnosed malignant disease in up to 20% to 40% of patients. Fine-needle biopsy has shown good performance for the diagnosis of metastatic nodules. The prognosis and the option of surgical treatment depend on the local control of the primary condition and the state of the associated systemic disease, therefore it must be individualized. In general, up to 80% of patients with thyroid involvement have multi-organ metastatic disease and surgical treatment is intended to be palliative to prevent complications resulting from local extension of the disease to structures of the upper aerodigestive tract in the neck. A case series of six patients with metastatic lesions to the thyroid gland with primaries in the kidney, breast and melanomas is presented below

The evolution of the diagnosis of infectious diseases began with the observation of the morphological characteristics of organisms such as ascaris and whipworms, followed by the use of the microscope and haematoxylin and eosin stains, which allowed recognition of microscopic characteristics undetectable with the naked eye, such as the viral cytopathic changes of herpes and the presence of fungi. Patterns of acute and chronic granulomatous inflammation were also observed; these were not specific to the exact aetiology of the disease, which led to the introduction of special methenamine stains for fungi and Ziehl-Neelsen for fungi and mycobacteria. Later, the use of immunohistochemistry was introduced, which acknowledged the use of antibodies to classify microorganisms and detect cases that were either difficult to interpret or in the midst of severe inflammatory processes. Currently, the use of molecular biology has made it possible to reach diagnoses that would have been very difficult to obtain through traditional methods; these techniques show key specific characteristics and facilitate the diagnosis of various infectious pathologies. These new techniques are based on the detection of antigens and nucleic acids of microorganisms, an important advance in the diagnosis of infectious diseases.

La enfermedad por coronavirus SARS-CoV-2 que surgió en el año 2019 (COVID-19), ha obligado al rápido desarrollo de vacunas para prevenir su propagación e intentar controlar la pandemia. Dentro de las vacunas desarrolladas, las primeras en ser aprobadas con una tecnología nueva en el campo de la vacunación, fueron las vacunas basadas en ARNm (ácido ribonucleico mensajero), que lograron tasas de efectividad cercanas al 95 % para la prevención de la enfermedad COVID-19 grave. Los eventos adversos comunes son reacciones locales leves, pero ha habido varios informes de pacientes que desarrollaron tiroiditis subaguda y disfunción tiroidea después de recibir la vacuna contra SARS-CoV-2. Este artículo presenta dos casos de tiroiditis subaguda poco después de recibir la vacuna contra COVID-19


The SARS-CoV-2 coronavirus disease which emerged in 2019 (COVID-19), has forced the rapid development of vaccines to prevent the spread of infection and attempt to control the pandemic. Among the vaccines developed, one of the first to be approved with a new technology in the field of vaccination, was the mRNA (messenger ribonucleic acid) vaccine, with rates of effectiveness close to 95% for the prevention of severe COVID-19 disease. Common adverse events are mild local reactions, but there have been some reports of patients developing sub-acute thyroiditis and thyroid dysfunction after receiving the SARS-CoV-2 vaccine. This article presents two case reports of subacute thyroiditis shortly after receiving the COVID-19 vaccine

RESUMEN La tuberculosis es una enfermedad infecciosa y frecuente en países en vía de desarrollo. Esta puede causar una amplia variedad de complicaciones y presentaciones atípicas con alta morbimortalidad. De la forma genitourinaria se sospechada muy poco, razón por la cual su diagnóstico se hace, usualmente, de forma tardía o no se realiza. Esto conlleva a consecuencias muy graves en los pacientes, por ejemplo, la enfermedad renal crónica terminal. A continuación, se presenta un reporte de caso de una paciente con la anterior enfermedad, secundaria a una tuberculosis renal bilateral diagnosticada tardíamente y se realiza una revisión de la literatura sobre este tema.


SUMMARY Tuberculosis is a common infectious disease in developing countries, which can cause a variety of complications and atypical manifestations with high morbidity and mortality. The urogenital form is rarely suspected, resulting in delayed diagnosis or even no diagnosis, which can have serious consequences for the patients, such as chronic end-stage renal disease. We report on a patient with chronic end-stage renal failure caused by a delayed diagnosis of bilateral renal tuberculosis and a literature review on this topic.

Malakoplakia is a granulomatous disease associated with an infectious etiology, usually involving the urinary tract. It reveals itself as a recurrent urinary tract infection (r-UTI), and in some cases, it is associated with impairment of renal function. Immunosuppression is one of its main associated factors, and it has been increasingly described in patients with solid organ transplantation (SOT), mainly kidney transplantation. Macroscopically, it can form masses and sometimes it may be confused with neoplasia, which is why histological findings are fundamental for the diagnosis. Here, we present a case of bladder malakoplakia, manifested by r-UTI from Escherichia coli in a patient with renal transplantation, refractory to long-term antibiotic treatment and reduction in immunosuppression, which resolved after surgical management. We also summarize the clinical characteristics of malakoplakia and compare them with previous reports in the literature on SOT.

BACKGROUND: Parathyroid cyst is an infrequent and unsuspected disease. There are more than 300 hundred cases reported in the world literature, a few of them are from Latin America. The experience of our centers and a review of the cases are presented. METHODS: Case report of a series of patients with parathyroid cyst from our institutions according to the CARE guidelines (Case Reports). A search of Medline, Embase, BIREME (Biblioteca Regional de Medicina) LILACS (Literatura Latinoamericana y del Caribe en Ciencias de la Salud), Google Scholar and Scielo (Scientific Electronic Library on Line) databases and telephonic or email communications with other experts from Latin-America was performed . RESULTS: Six patients with parathyroid cyst were found in our centers in Colombia. Most of them were managed with aspiration of the cyst. Two of them required surgery. Only one case was functional. Twelve reports from Latin America were found for a total of 18 cases in our region adding ours. CONCLUSIONS: Parathyroid cysts are uncommonly reported in Latin America. Most of them are diagnosed postoperatively. Suspicion for parathyroid cyst should be raised when a crystal clear fluid is aspirated from a cyst. The confirmation of the diagnosis may be easily done if parathyroid hormone (PTH) level is measured in the cyst fluid.

Neoplasms occur naturally in invertebrates but are not known to develop in tapeworms. We observed nests of monomorphic, undifferentiated cells in samples from lymph-node and lung biopsies in a man infected with the human immunodeficiency virus (HIV). The morphologic features and invasive behavior of the cells were characteristic of cancer, but their small size suggested a nonhuman origin. A polymerase-chain-reaction (PCR) assay targeting eukaryotes identified Hymenolepis nana DNA. Although the cells were unrecognizable as tapeworm tissue, immunohistochemical staining and probe hybridization labeled the cells in situ. Comparative deep sequencing identified H. nana structural genomic variants that are compatible with mutations described in cancer. Invasion of human tissue by abnormal, proliferating, genetically altered tapeworm cells is a novel disease mechanism that links infection and cancer.

The primary bilateral macronodular adrenal hyperplasia or the independent adrenocorticotropic hormone bilateral nodular adrenal hyperplasia is a rare cause hypercortisolism, its diagnosis is challenging and there is no clear way to decide the best therapeutic approach. Adrenal venous sampling is commonly used to distinguish the source of hormonal production in patients with primary hyperaldosteronism. It could be a useful tool in this context because it might provide information to guide the treatment. We report the case of a patient with ACTH independent Cushing syndrome in whom the use of adrenal venous sampling with some modifications radically modified the treatment and allowed the diagnosis of a macronodular adrenal hyperplasia.


La hiperplasia macro nodular bilateral o hiperplasia adrenal nodular bilateral independiente de la hormona adrenocorticotrópica es una de las causas menos frecuentes de hipercortisolismo, su diagnóstico supone un reto y no se tiene claridad de cuál es la mejor aproximación terapéutica. El muestreo venoso adrenal que frecuentemente se utiliza para hacer la distinción del sitio de producción hormonal en el hiperaldosteronismo primario podría ser una herramienta útil en este contexto ya que podría brindar información que pudiera guiar el tratamiento. Presentamos el caso de una paciente con síndrome de Cushing ACTH independiente en quien el uso del muestreo venoso adrenal con algunas modificaciones cambio de manera radical el tratamiento y permitió confirmar una hiperplasia adrenal macro nodular.

Prostate cancer (PCa) is currently diagnosed by microscopic evaluation of biopsy samples. Since tissue assessment heavily relies on the pathologists level of expertise and interpretation criteria, it is still a subjective process with high intra- and interobserver variabilities. Computer-aided diagnosis (CAD) may have a major impact on detection and grading of PCa by reducing the pathologists reading time, and increasing the accuracy and reproducibility of diagnosis outcomes. However, the complexity of the prostatic tissue and the large volumes of data generated by biopsy procedures make the development of CAD systems for PCa a challenging task. The problem of automated diagnosis of prostatic carcinoma from histopathology has received a lot of attention. As a result, a number of CAD systems, have been proposed for quantitative image analysis and classification. This review aims at providing a detailed description of selected literature in the field of CAD of PCa, emphasizing the role of texture analysis methods in tissue description. It includes a review of image analysis tools for image preprocessing, feature extraction, classification, and validation techniques used in PCa detection and grading, as well as future directions in pursuit of better texture-based CAD systems.

Se describe el caso de una paciente evaluada a los 29 años por un tumor tiroideo, clasificado inicialmente como un carcinoma insular de tiroides; presentaba gran compromiso tumoral en cuello, mediastino y pulmones. Tres años después de recibir tratamiento específico para su neoplasia tiroidea, desarrolló un síndrome de Cushing y lesiones hepáticas sugestivas de metástasis del tumor primario. Al revisar la patología se encontró, en lo que inicialmente se había clasificado como un carcinoma insular, un carcinoma medular de tiroides productor de hormona adrenocorticotrópica (ACTH). Con base en este caso, se presenta una revisión de la literatura médica sobre el tema.


Cushing syndrome secondary to a medullary thyroid carcinoma: report of a case and review of the literature We report the case of a 29-year old female who was evaluated because of a thyroid tumor. The initial pathological classification was an insular thyroid carcinoma. There was strong involvement in the neck, mediastinum and lungs. Three years after receiving specific therapy for her thyroid neoplasia, she developed a Cushing syndrome and liver lesions suggestive of metastases from the primary tumor. A review of the previous pathological material revealed a medullary thyroid carcinoma producing ACTH, instead of the insular carcinoma. Based on this case a review of the literature is presented.

En este artículo se compila brevemente la biografía de Carlos Juan Finlay, considerado el Pasteur latinoamericano y el más importante investigador de la fiebre amarilla. Carlos Finlay nació en 1883 en la ciudad de Puerto Príncipe, Cuba, estudió en el Jefferson Medical College en Filadelfia, donde completó la carrera de medicina en 1855. En 1881, describe por primera vez que la transmisión de la fiebre amarilla se produce por la picadura de un mosquito y 20 años después, gracias a su colaboración a la Comisión Americana de la Fiebre Amarilla, se demuestra el rol del mosquito como vector y se instauran prácticas de erradicación del insecto. Posteriormente, Finlay se dirige a Panamá para ayudar a combatir las enfermedades infecciosas que atacan a los trabajadores de la construcción del canal. El 19 de Agosto de 1915 fallece en su casa de Vedado en la Habana, Cuba.


This article summarise briefly the biography of Carlos Juan Finlay, who is considered the Latin American Pasteur and the most important researcher of yellow fever. Carlos Finlay was born in 1883 in Puerto Principe, Cuba. He studied at Jefferson Medical College in Philadelphia, where completed a degree in medicine in 1855. At 1881, describes for the first time that the transmitting of yellow fever is cause by the bite of a mosquito, and 20 years later, contributes to the American Commission of Yellow Fever, showing the role of mosquito on the disease, guiding practical methods to eradicate yellow fever. Subsequently, Finlay went to Panama to help to fight the infectious diseases in workers who built the canal. The August 19, 1915 died at his home in Vedado in Havana, Cuba.

Las recomendaciones para la biopsia por aspiración con aguja fina de mama se desarrollaron y aprobaron en 1997 por el Instituto Nacional de Cáncer en Bethesda, Estados Unidos y fueron adaptadas a nuestro país en 2007, sin embargo, en los últimos años no se han realizado cambios formales en estas indicaciones. El objetivo de este módulo es presentar la actualización del reporte de biopsia por aspiración con aguja fina de mama, usando el sistema de reporte Bethesda, realizado por consenso con un grupo de patólogos, clínicos, radiólogos, cirujanos de mama y otros profesionales de la salud de Colombia y otros países, y con base en la experiencia realizando biopsia por aspiración con aguja fina de mama del Hospital Pablo Tobón Uribe y de Dinámica IPS.


Recommendations for breast fine needle aspiration biopsy were developed and approved in 1997 by The National Cancer Institute of Bethesda, United States, , and were adapted to our country on 2007, however, in last years these indications have not changed in a formal manner. The purpose of this review was to provide an update of the report for breast fine needle aspiration biopsy using the Bethesda system. This guide was made by consensus with pathologists, clinicians, radiologists, breast surgeons and other health professionals of Colombia and other countries. En este artículo se presenta un consenso médico basado en el sistema de Bethesda del Instituto Nacionalde Cáncer (Estados Unidos) para el uso de la biopsia por aspiración con aguja fina en el manejo de nódulos tiroideos, realizado en conjunto con patólogos, radiólogos, endocrinólogos y otras especialidades médicas de Colombia, España, Chile, Venezuela, Estados Unidos y Panamá. En este trabajo se describen las indicaciones de la biopsia por aspiración con aguja fina de tiroides, requisitos previos, entrenamiento, acreditación, técnicas, terminología diagnóstica, pruebas complementarias y opciones de tratamiento. El objetivo del actual artículo es presentar ante la comunidad médica la clasificación de los reportes citológicos, el reporte de ecografía que propone usar el sistema de datos y el reporte de imágenes tiroideas (TIRADS, del inglés The Thyroid Imaging Reporting and Data System), el uso de la medición de tiroglobulina en biopsia por aspiración con aguja fina y técnicas de citología líquida;...


This article presents a medical consensus based on the Bethesda system of the National Cancer Institute (USA) for the use of fine needle aspiration biopsy in the management of thyroid nodules. This consensus was performed in conjunction with pathologists, radiologists, endocrinologists, and other medical specialties of Colombia, Spain, Chile, Venezuela, United States, and Panama. In this work was described the indications for fine needle aspiration biopsy of thyroid, prerequisites, training, accreditation, techniques, diagnostic terminology, additional tests and treatment options. La identificación de microorganismos en tejido es esencial para reconocer un proceso infeccioso. Inicialmente, mediante la coloración de hematoxilina-eosina, se puede identificar el patrón de inflamación asociado y luego, a través de tinciones basadas en plata y la tinción de Gramde tejido se visualizan los microorganismos. La tinción de Gram no solo sirve para bacterias, sino también para algunos hongos y parásitos; no obstante, esta técnica tiene algunos inconvenientes, como la contaminación con otros microorganismos y la imposibilidad de visualizar algunas bacterias, entre ellas Legionella pneumophila, Leptospira spp y Bartonella spp. En este artículode revisión se describirán los fundamentos del Gram de tejido, su contribución en el diagnóstico de infecciones como herramienta adicional para el reconocimiento de microorganismos, y sus limitaciones.


The identification of microorganisms in tissue is pivotal to recognize infectious processes.At first, the hematoxylin-eosin stain is used to identify the pattern of inflammation associated; after that, microorganisms are seen through Gram or silver stains. Gram Stain of tissue biopsy not only stains bacteria, but also a number of fungus and parasites. However, this technique has some disadvantages, such as contamination with other microorganisms, and lack of stain of some bacteria, including Legionella pneumophila, Leptospira spp and Bartonella spp. This review articleaims to describe the fundamentals of Gram stain of tissue biopsy and its assistance in infectious diagnosis as an additional tool for recognition of microorganisms, as well as its limitations.

RESUMO

Las metástasis por melanoma a la glándula tiroides son raras, su presentación representa para elpaciente un pésimo pronóstico, dado que indica enfermedad diseminada terminal. El proceso de diagnóstico no difiere al usual para toda masa tiroidea y su tratamiento final depende del estado del paciente y de lo avanzado de la enfermedad. En el presente artículo presentamos dos casos de melanomametastásico a la tiroides.


Metastatic melanoma to the thyroid gland are rare, their presence on a patient represents bad prognosisas it indicates disseminated terminal disease. The diagnostic process is similar to all thyroid masses and its final treatment depends on the patient´s status and how disseminated the disease is. In the present article we describe two cases of metastatic thyroid melanoma.

El asesinato del presidente John F. Kennedy, el 22 de noviembre de 1963 a las 12:30 p.m., ha sido considerado el crimen del siglo XX. Se hace un recuento de los hallazgos de la atención primaria en el Parkland Memorial, la autopsia y hallazgos radiológicos descritos por los médicos y publicado en la revista J.A.M.A.


The murder of the president John F. Kennedy, on Novermber 22nd of 1963 at 12:30 P. M. has been regarded as the crime of the XX century. An account is presented about the primary care findings at Parkland Memorial as well as the autopsy and radiologic findings described by physicians and published in the JAMA magazine.

RESUMO

La Biopsia por Aspiración con Aguja Fina (BACAF) es el método de elección en el diagnóstico de nódulos tiroideos, mamarios, ganglios linfáticos y glándulas salivares. En piel y tejidos blandos ha sido utilizada con poca frecuencia. Presentamos dos casos de endometriosis extrapélvica, localizada en tejidos blandos y pared abdominal. Se trata de dos mujeres con nódulos dolorosos en la pared abdominal y sobre cicatriz de cirugía ginecológica. El diagnóstico se confirmó en una de las pacientes con biopsia excisional, otra recibió tratamiento médico.


Fine-Needle Aspiration Biopsy is the method of choice for the diagnosis of thyroid, breast, lymph and salivary glands nodules; nevertheless, it’s use has been poor regarding sking and soft tissues evaluation. In the following paper, the authors present two cases of extrapelvic endometriosis, located on soft tissues and abdominal wall, in two women with tender nodes on abdominal wall and over a gyneacologic surgery scar. Diagnosis was confirmed on one of the patients via excisional biopsy; the other one underwent medical management.
